On August 31, 2019, at the Minneapolis Armory in Minneapolis, Minnesota, Sánchez faced Victor Bisbal for the vacant WBO-NABO heavyweight title after accumulating six additional victories, four of which came by way of knockout. After Bisbal’s corner notified the referee Celestino Ruiz that Bisbal was unwilling to continue before the start of the fifth round, Sánchez defeated Bisbal via fourth-round corner retirement (RTD). Sánchez was inserted as a last-minute replacement after Efe Ajagba withdrew from a match against Jack Mulowayi due to a back injury received during training. The fight happened on October 26 at Reading, Pennsylvania’s Santander Arena.
With a score of 100-90 from all three judges, Frank Sanchez triumphed by unanimous decision (UD) to win the vacant WBO-NABO title for a second time. He fought Joey Dawejko on March 7, 2020, at the Barclays Center in New York City for the vacant WBC Continental Americas heavyweight title. Before facing Nagy Aguilera on the undercard of Canelo lvarez vs. Billy Joe Saunders on May 8, 2021, Sánchez defeated Brian Howard and Julian Fernandez by knockouts following his victory over Dawejko.
After six rounds, Sánchez prevailed via unanimous technical decision after Aguilera informed authorities that he was unable to continue due to an unintended foul. On October 9, 2021, Sanchez fought the unbeaten Efe Ajagba as part of the Tyson Fury vs. Deontay Wilder III undercard. On his way to winning by unanimous decision with scores of 98-91, 98-91, and 97-92, Sánchez knocked Ajagba to the ground. On January 1, 2022, Sánchez was scheduled to fight Carlos Negrón as the co-feature of the heavyweight fight between Luis Ortiz and Charles Martin.
Negrón withdrew from the fight on December 28, 2021, citing a positive COVID-19 test, and Christian Hammer would take his place. Frank Sanchez prevailed via unanimous decision in the match. Every round of the fight was given to him, and in the tenth, he scored a questionable knockdown.
